The authoring turn (a Director turn or a paid re-authoring op) finished with an error while the call waited for it. `extensions.reason` is the typed cause when there is one (`insufficient_balance`: the wallet no longer covered the turn when it started; `not_found`: the project, its version or its user were gone), `extensions.runId` the job, and `extensions.outcome` is `failed_inline`. The turn is charged when its result is written, so one that fails before that costs nothing; repeating the request starts a new turn.
